Canyonlands Healthcare offers comprehensive one-stop services and a health care home for patients in each of the communities served by the organization. Our Health Centers provide more than just treatment for illness or episodic conditions. We provide ongoing care and health management for families and individuals throughout their lives.

Canyonlands Healthcare is a private, not for profit, corporation providing consumer-directed health centers which provide high quality, cost-effective and comprehensive primary and preventive care. Canyonlands offers special programs designed to improve access to care for those who face financial barriers and need assistance. Canyonlands Healthcare has been designated as a Federally Qualified Health Center (FQHC) and as such is supported by federal grants under the U.S. Public Health Service Act.

Message from our CEO

Canyonlands Healthcare has the honor of serving Arizona communities for close to 40 years. Since our formative days as a single facility in Page, to the present where we serve 7 communities and over 18, 000 users, our commitment to improving the health of our patients, their families and the community have never faltered. Canyonlands dedicated staff of professionals is here to provide comprehensive medical and dental care to you and your family through a patient-centered, integrated medical home model.

Canyonlands services are available to anyone in the area's we serve. We offer on-site financial counselors who work closely with our patients to ensure they have access to all available financial resources.

Canyonlands has over 100 outstanding employees that are dedicated to meeting the needs of our patients and the communities we serve. Canyonlands supports local communities through health fairs, educational seminars, school-based dental programs, and partnerships with other local health providers and systems. Our employees are dedicated to supporting our patients both in and outside of our facilities.

Canyonlands Healthcare uses state of the art technology in the delivery of care. We have deployed electronic medical and dental records, including digital dental imaging. Our products are designed to improve the patient experience and deliver the highest quality of care. We have plans for integration of telemedicine to further support our rural locations. Our service delivery models are research-based, and align with the most current treatment modalities.

Canyonlands is governed by a consumer driven Board of Directors that is committed to ensuring that our centers remain the focal point of service in our communities.
