Canyonlands Community Health Care

Offers comprehensive one-stop services and a health care home for patients in each of the communities they serve.  Our Community Health Centers provide more than just treatment for illness or episodic conditions.  We provide ongoing care and health management for families and individuals throughout their lives. Canyonlands Community Health Care centers are private, not for profit (charitable donations and sponsorships), consumer-directed health care centers which provide high quality, cost-effective and comprehensive primary and preventive care to all people in the areas they serve with special programs for the medically underserved and uninsured people.  Our regional network of health centers are supported by federal grants under the U. S. Public Health Service Act and the State of Arizona Primary Care Program.
 

Nationwide Mission of Community Health Centers

Currently there are over 1,000 community health centers serving in 3,600 communities.  They are caring for over 15 million patients nationwide.  The need for community health centers is great.  There are 45 million Americans uninsured and the proportion of Americans without insurance has been steadily rising since the year 2000.  These community-based providers are also commonly referred to as Federally Qualified Health Centers (FQHCs) because they meet rigorous federal standards related to quality of care, as well as cost effectiveness.  They are qualified to receive cost-based reimbursement under Medicaid and Medicare law.
