Vaccines for Children

Canyonlands Community Health Care is a participant in the Arizona Department of Health Services Vaccines for Children Program. Vaccines for Children provides FREE IMMUNIZATIONS for eligible children birth through 18 years old. Your child’s immunization records will be included in the Arizona State Immunization Information System (ASIIS). An official immunization history can be computer-generated for parents to receive a free copy for schools, day care, camp, or college requirements. The Vaccines for Children program provides all childhood and adolescent immunizations that are recommended by the Centers for Disease Control and the American Academy of Pediatrics. Families who have insurance plans that fully cover immunizations are not eligible for free Vaccines for Children immunizations. Please contact your insurance provider to verify immunization benefits prior to your visit. Arizona School and Child Care Immunization Laws recommend that children be immunized against vaccine-preventable diseases. Come to any of our sites to ensure that your children and your community are protected against serious infectious diseases. ** Parents-do you know if your child is up to date on school immunizations? Bring your child's immunization record to any of our sites and our caring staff will let you know if your child is due for vaccinations that protect against these diseases: Tetanus, Diphtheria, Polio, Measles, Mumps, Rubella, Haemophilus influenza type b, Hepatitis B, Hepatitis A, Varicella, Meningococcal, Influenza and Pneumococcal. |
