Canyonlands Community Health Care

Canyonlands Community Health Care is a participant in the Arizona Department of Health Services Vaccines for Children Program. Vaccines for Children provides FREE IMMUNIZATIONS for eligible children birth through 18 years old. Your child’s immunization records will be included in the Arizona State Immunization Information System (ASIIS). An official immunization history can be computer-generated for parents to receive a free copy for schools, day care, camp, or college requirements.

The Vaccines for Children program provides all childhood and adolescent immunizations that are recommended by the Centers for Disease Control and the American Academy of Pediatrics. Families who have insurance plans that fully cover immunizations are not eligible for free Vaccines for Children immunizations. Please contact your insurance provider to verify immunization benefits prior to your visit. Arizona School and Child Care Immunization Laws recommend that children be immunized against vaccine-preventable diseases. Come to any of our sites to ensure that your children and your community are protected against serious infectious diseases.

Bring your child's immunization record to any of our sites and our caring staff will let you know if your child is due for vaccinations that protect against these diseases: Tetanus, Diphtheria, Polio, Measles, Mumps, Rubella, Haemophilus influenza type b, Hepatitis B, Hepatitis A, Varicella, Meningococcal, Influenza and Pneumococcal.

Why Immunize?

Children in child care and school settings are at increased risk for sharing the germs that cause serious vaccine-preventable diseases.

Before vaccines:

Infectious diseases killed thousands of children in the United States each year.
Diphtheria: 100,000-200,000 cases of Diphtheria killed 13,000 people each year. (D in DTaP)
Pertussis: 175,000 cases of Pertussis (whooping cough) killed 8,000 children each year. (P in DTaP)
Polio: One epidemic killed 6,000 people and paralyzed 27,000.
Measles: More than 3 million cases of measles and 500 deaths from measles occurred each year. (first M in MMR)
Mumps: More than 100,000 cases each year. Approximately 10,000 cases involved meningitis (inflammation of the covering of the brain and spinal cord). (second M in MMR)
Rubella: One rubella (German measles) epidemic with 12.5 million cases caused 20,000 infants to be born with congenital rubella syndrome. 11,600 were deaf, 3,580 were blind, and 1,800 were mentally retarded because of the infection. (R in MMR)
Haemophilus influenza type b: Hib caused pneumonia, meningitis, and other serious infections in 20,000 children each year
